Personal Particulars Update Form

I have read and agreed to the following:

1. I hereby authorise Singapore Teachers Co-operative Society Ltd (hereinafter referred to as the “STCS”) to update my personal particulars.
2. I hereby declare and confirm that the personal data that I have provided on this form is accurate and complete.
3. I consent to my personal data being collected, used, and retained by STCS for the purpose of processing, administering, and managing my Co-operative membership.
4. STCS will not update my new particulars if the required supporting document is missing.
5. I further understand that STCS reserves the right to decline to fulfil this Personal Particulars Update request as permitted under the Personal Data Protection Act 2012.
